چکیده

Background: In major orthopedic surgery, prevention of venous thromboembolism has been based on Unfractionated Heparins (UFHs) over the past decades, then on LowMolecular Weight Heparins (LMWHs), and on New Oral Anticoagulants (NOACs) more recently. To summarize the comparative effectiveness of UFHs, LMWHs, and NOACs in this clinical indication, we applied Bayesian network meta-analysis to the clinical material (randomized studies) published in two previous reviews focused on this issue. Objectives: Our end-point was a composite of venous thromboembolism and pulmonary

Direct thrombin inhibitors for the prevention of venous thromboembolism after major orthopaedic surgery.

Unfractionated heparin, low-molecular-weight heparins and vitamin K antagonists are established antithrombotic agents, but all have a number of limitations. Unfractionated heparin requires parenteral administration, has a short half-life and variable dose-response relationship. Low molecular weight heparins are more effective than low-fixed-dose unfractionated heparin in the prevention of posto...

Prevention of venous thromboembolism in internal medicine with unfractionated or low-molecular-weight heparins: a meta-analysis of randomised clinical trials.

BACKGROUND The prevention of venous thromboembolic disease is less studied in medical patients than in surgery. METHODS We performed a meta-analysis of randomised trials studying prophylactic unfractionated heparin (UFH) or low-molecular-weight heparin (LMWH) in internal medicine, excluding acute myocardial infarction or ischaemic stroke. Deep-vein thrombosis (DVT) systematically detected at ...

Differentiation of parenteral anticoagulants in the prevention and treatment of venous thromboembolism

BACKGROUND The prevention of venous thromboembolism has been identified as a leading priority in hospital safety. Recommended parenteral anticoagulant agents with different indications for the prevention and treatment of venous thromboembolism include unfractionated heparin, low-molecular-weight heparins and fondaparinux. Prescribing decisions in venous thromboembolism management may seem compl...

Role of Novel Oral Anticoagulants in Primary and Secondary Thromboprophylaxis in Cancer

The development of venous thromboembolism is one of the major causes of morbidity and mortality in patients with cancer. The standard treatment of deep venous thrombosis or pulmonary embolism occurring in patients with active malignancy remains the use of low molecular weight heparins. However, in clinical practice, practitioners are frequently asked about the efficacy and safety of various new...

Fondaparinux: a different weapon in the war on clot: update for the clinical nurse specialist.

Despite the chemical arsenal available for thrombosis prevention, unfractionated heparin (UFH), and lowmolecular-weight heparins (LMWHs), clot remains a major threat for medical and surgical patients. In the absence of prophylaxis, venous thromboembolism (VTE) occurs in 10% to 40% of medical and general surgery patients and in 40% to 60% of major orthopedic patients. Heparin was discovered by c...
